I guess I'm missing something obvious, but how do i get a menu entry to open a command prompt?

Just as you add any other item to a Regular menu:

Open the NextSTART menu, right click on the menu item where you want to insert the shortcut to the command prompt and select 'Insert'.

In the 'New Item Properties' dialog that pops up, type in 'Command Prompt' in the Label field, make sure the pull up menu says 'Run Program' and type cmd.exe in the Filename field. Specify an icon if you want to and click Ok.

Alternatively, locate the cmd.exe file in the Windows\System32\ folder and drag & drop it into the NextSTART menu.
