I use Cursor FX Plus but there may well be other solutions.

It comes in two versions, a free one with a subset of features and a full featured paid for version.

The full featured version currently costs $US 19.95.

Although the Stardock site states you have to download impulse in order to download the full featured version I don't believe that is the case anymore, in fact entirely the opposite. My understanding is that Stardock are phasing out the Impulse requirement for downloading stardock 'desktop' applications and you should be provided with a link to download the installation file.

I'm afraid I have never investigated designing cursors, I just use cursors designed by others! Here is a directory of cursors that can be downloaded in addition to those built in the the installation file.
